How to Register?
Call (936) 500-4767 to make your appointment. You can choose a day and time that works for you. Once you complete your intake, future visits go quickly.
Items Needed for Registration
Bring these documents on your first visit:
- Proof of your current address, such as a utility bill dated within the last 90 days
- A Texas Driver's License or Texas ID for every adult in your household
- Social Security cards for everyone in your household
